Statement of Product Cleanliness and Decontaminations

In our effort to protect the health of our employees and provide a safe working environment, Onset recommends that all products returned for service are clean and uncontaminated by harmful substances such as hazardous materials, chemicals, insects, and pests.  For a list of compatible cleaning methods, see the Product Cleaning Reference Guide.

If you are returning any product, you must complete this form to describe the environment from which the equipment was taken, any hazardous substances to which the equipment was exposed, and any cleaning and/or decontamination processes that were performed.

Please submit this form or fax the completed form to 508-759-9100 to the attention of Technical Support.  Onset Corporation reserves the right to refuse service on product where specific applications may cause health and safety concerns for its employees.
